I think the best way to batch-extract cover-art from MP3s into image files is using this tool I wrote: Set/Clear/Extract WMA/MP3 Cover-art + Populate AlbumArtist.
(The same tool can also do the reverse, but Opus itself is probably better at that now. It was written before Opus had that functionality.)
