Folders being duplicated in tree after folder name and/or contents modified

Attached are before and after screenshots showing, for example, a folder named "Cedar Cove 10 2010 - 1022 Evergreen Place". The first screenshot shows only one folder with that name, the second shows two folders with the same name. The duplication occurred after I had added content to the original folder. The contents of the folders is the same. If I delete the second folder, both of them disappear. Using the "Edit Undo" function, the folder will be restored, only one folder, not two.

PS: When I close the program using the X in the top RHS and reopen, the duplicates have disappeared.

What type of drive/folder is that? E.g. Local drive, network drive, cloud storage, USB stick, phone/tablet/MTP etc.

Is the action that triggers the extra folder to appear done in Opus, or is it how Opus is responding to external changes? (Or does it happen with both?)
