I use to replace the folder icons with other icons of my choice.
Although the icons appear in the tree and anywhere alse, they won't appear in the tabs. I searched and looked in the preferences but no luck. Is this a known problem of Opus?
Something else: If I rotate an image with the opus viewer it won't be auto-saved like in MS-Explorer on XP. Any way to fix that?

To see the changes you've made to your system icons, you need to exit Dopus completely (meaning: close all dopus windows, and choosing exit from the tray icon) first. Starting it again should now have it display your icons correctly.
As for saving the rotated images in the viewer, you need to actually choose save from the file menu.

Well I use Opus for a couple of months now and I have had these icons since windows explorer without changing them when I changed to opus.
Do I have to change the icons all over again, so that opus can recognise and use them in the tabs?

It might help, as windows quite frequently messes up the icon cache. You could try using Microsoft's TweakUI to repair it (you can find it under the repair tab in the application).
As you can see from my attached screenshot, it is possible though
