When I right click on the column headings to add additional columns and go to the submenu and choose other, keywords is listed twice as a column name. If I choose the first occurrence it terminates the program, the second occurrence appears to work properly.
Which category is it listed under? Is it the "Other" category?
The columns are probably both coming from shell extensions, one of which isn't written very well.
I can confirm this! The two "Keyword" entries are also there in my german version. They are not translated to german and one of them crashes DOpus without any messages or a Dumpfile created. They occur under "Others".
It seems to come from Open Office. With Nirsoft ShellExView I found two entries for Column Handlers. One coming from PDF XChange PDF-Viewer and one from OpenOffice. I deactivated the Open Office entry and now I have only one working Keyword entry.

(I am still bitter about all the problems their ActiveX control has caused me, before giving up on it entirely as they clearly have as well.)
Should I report this as a problem?
Which OS are you on? If it's XP, try adding the same column to Explorer to see if it crashes as well. (If it's Vista/7, Microsoft removed the column support from Explorer so you can't try that.)
Either way, the crash will almost certainly be due to a bug in the shell extension that adds the column, so if you want to report it you should report it to the authors of the shell extension. In all likelihood, the most that Opus could do is blacklist the extension so it doesn't appear at all.
I'm on win7x64. I know there was reference to OpenOffice in his thread but how do I know that's the cause?
Use NirSoft's ShellExView as Kundal suggested.
It will show you all the shell extensions installed on your computer. Look at the Column Handler ones (I think that's the name; it'll have "Column" in it if not), try disabling some & restarting Opus to see if the offending column goes away from the list..
This is what I see in ShellExView:
Right-click the entry and choose "Disable Selected Items"
