Right click on the first entry of Documents.
That looks correct.
It might be worth running the debugging steps shown here, making a log of what happens when the problem occurs, and another log at a time when the problem is not happening, to compare the two:
That may reveal something unusual/unexpected about the change events which are being generated on your system.
