Fixed icon browse dialog category list containing duplicated categories.

Fixed rename suggestion list auto-inserting if you used shift+down, closed the list and then pressed ctrl+space.

Rename suggestion list no longer auto-selects the current filename when it opens if it appears in the list.

Possible fix for crash IDs 301, 302, 303, 304, 306 and Meg Macintyre crash.
