Have you opened the existing toolbars via a self-made command which uses the LOCAL flag to make them local to the lister? The list in the customize dialog will only show checks for Global toolbars. Not sure what happens if you turn one on globally when it's already a local toolbar in one of the current listers...
If you exit Customize mode, then right-click an empty area on a toolbar and select Toolbars in the pop-up menu, does that list work?
If not it may be a file/registry permissioning issue. You may be able to work around it by toggling Preferences / Misc. / Misc.: Change configuration mode... and restarting Opus, although it's worth getting to the bottom of what's wrong even if that does seem to make things work.
Note: To restart Opus, first exit it completely using the tray icon.
I think a settings backup (use the Settings > Import & Export... wizard so you get everything), then a complete uninstall/clean (e.g. delete the GPSoftware registry keys under HKLM and HKCU if you're familiary with registry editing), then a reinstall is the best thing to try.
Check that the problem is gone before importing the saved settings, in case something within them is the trigger.
Let us know how you get on, and if the settings file triggers the problem then it's something GPSoft would probably like to take a look at.

Sorry I don't have a magic wand, but something's obviously corrupted or wrong with your installation.
It takes about a minute to uninstall, delete the registry keys, then reinstall, and a further few seconds to import your backup after checking whether or not a clean install exhibits the problem.
